Membership in the Shadow Hills Men's Golf Club

Membership in the Shadow Hills Men's Golf Club runs from January 1 through December 31 of each year and must be established annually by completeing an application package consisting of an "Application Form", a "Waiver Aggreement" and a "Profile Questionnaire".  These forms may be obtained as a package by clicking below.

Follow the instructions on the forms to complete.  Note that a US bank check and signed Waiver Agreement must accompany the Application Form.
